The rate of tendon injuries has increased significantly. Often, apparent cases of tendinitis are resistant to conventional management aimed at reducing the inflammatory process. This leads one to wonder if approaching these entities from an inflammatory model is appropriate. It is unclear whether acute tendinitis, chronic tendinitis, and tendinosis exist on a continuum or if they are independent entities. At this point in time we are unable to correlate histologic state with clinical presentation. This article is a critical review of the literature with respect to the etiology, histopathology, clinical presentation, and treatment of tendinopathies that are commonly present in the clinical setting. Future research is needed to correlate clinical signs and symptoms with tissue biopsies to help establish useful clinical guidelines. This would facilitate the selection of the most appropriate therapeutic management of the tendinous injury based on its true pathology.
